Combatant-General License Application
Checklist for Fighter Trainer and Cornermen
UNARMED COMBATANT APPLICATION REQUIREMENTS
This License Application must be filled out completely, accompanied by the following, and must be submitted no later than 4:30 p.m. on the day that begins 48 hours prior to a scheduled event:
- A passport-sized physical or digital color photo of the applicant. _____
- A MONEY ORDER or CASHIER’S CHECK* in the amount of $10.00 (Amateur) or $25.00 (Professional), payable to theArizona Department of Gaming (personal checks will not be accepted for the license application fee). _____
- A copy of the applicant’s driver’s license, valid identification, or other lawful presence documents. _____
- Boxer’s Federal Identification or National Mixed Martial Arts Identification application must be filled out completely. An identification card and number will be issued to you, which will serve as your identification to check-in at each event you participate in. This must be carried at all times to verify your identity. _____
- Annual physical exam (form available). Examinations must be performed by a MD or DO (examinations by chiropractors, nurse practitioners, and physician assistants will not be accepted). _____
- HIV test, hepatitis B (surface antigen) test & hepatitis C (antibody) test. _____
- Ophthalmological exam (form available). _____
- Concussion baseline testing (form available). _____
- Arizona is a calendar-year state for medical requirements, however an applicant may petition the Commission for waiver of medical licensing requirements upon renewal if the combatant fulfilled those requirements within 90 days prior toDecember 31.
- To obtain a license, an applicant over 32 years of age must get approval from the Executive Director and may be required to furnish additional medical tests. _____
- To obtain a license, an applicant over 36 years of age must get approval from the Commission and is required to furnish the results of a stress test that is administered by a licensed physician, accompanied by a clearance letter, and the results of an electrocardiogram (ECG) that demonstrates normal cardiovascular function. _____
- An applicant who has not been active for two years or more may be required to undergo further medical exams and/orbe asked to demonstrate ability to perform. _____
- A female applicant must submit a negative pregnancy test on the day of weigh-ins or date of event as determined by the Executive Director. _____
- An applicant who is making a pro debut must furnish: proof of the applicant’s amateur record; the name of the training facility; and an affidavit from the applicant’s trainer, certifying the applicant’s ability to perform. If the applicant has noamateur experience, he/she must furnish an affidavit from the applicant’s trainer certifying the applicant’s tenure in the gym and the ability to perform. _____
- The Commission may establish other requirements for each applicant, as the Commission deems necessary.
